Please note that all requests, including, but not limited to, transfers of interests and address updates, must be submitted in writing with (i) a cover letter stating your purpose, (ii) the appropriate support documentation, and (iii) a completed W-9. We are unable to make any changes to your ownership information over the telephone. For your convenience, please find below a chart which lists common situations that require a change to our ownership database, with examples of the type of documentation that are required to make such a change:

Company or Corporation Merger | Certification of Merger, including new Tax ID number submitted in writing to the address listed below. |
Appointment of Guardian or Power of Attorney | Copy of Power of Attorney or Letters of Guardianship to the address listed below. |
Sale of an Interest | Copy of Conveyance Instrument as filed for record in the county or parish where the property is located submitted in writing to the address listed below. |
Change in Ownership due to Divorce | Include Complete Divorce Decree including any conveyances made and filed for record in the appropriate county/parish submitted in writing to the address listed below. |
Transfer of Ownership to a Trust | Copy of Trust Agreement or Memorandum of Trust along with recorded conveyance in appropriate county/parish submitted in writing to the address listed below. |
Change in Ownership Due to Death of the Owner | Please submit in writing to the address listed below. Include death certificate for owner, Letters of Testamentary, copy of probated will (if applicable) or recorded Affidavit of Heirship. Please also include the current address for new owner/s. |
Request for Tax Payer–W9 | For all changes, please submit a W-9. |
